Patient Forms
Upon entering our office, we will request that you complete our new patient paperwork. This paperwork provides us with your health history and information on your condition. We also offer the paperwork to be completed online.
Consultation
Next, you will have a consultation with Dr. Nicholas Hagan to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Nicholas Hagan to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
Examination
After your consultation, Dr. Nicholas Hagan will perform a complete chiropractic examination. Dr. Hagan will assess for the underlying cause to your condition. Other standard neurological, orthopedic, postural, and physical tests will be performed if indicated.
Initial Correction
If you qualify as a candidate for our chiropractic care, you can choose to experience your first treatment. This includes your first adjustment. Treatment is interactive, so you can express concerns about the different styles of treatment anytime. At the end of the treatment, Dr. Hagan will provide you with a brief, verbal report of prognosis. A follow up appointment will then be scheduled to provide a full report and recommendations.
Professional Recommendations
On the second visit, Dr. Nicholas Hagan will give you a detailed report of all findings and answer any questions including:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, goals, and examining your nervous system, Dr. Nicholas Hagan will discuss recommendations and notify you if your condition requires care with other providers. Dr. Nicholas Hagan will provide the best treatment and wellness program for your needs.
